Forms for Agencies Outside the Executive Domain

• Equipment Pick-up Form- If you are prompted for a username and password just click cancel. Then click 'File' and 'Save As'.

• FOIA Request for Information - This form is the INTERNAL USE ONLY. Agencies are required to use this form to assist in the recovery of electronic data for FOIA requests. If you do not enter accurate or required information, we may be unable to fulfill your data request.

• Surplus Property Retirement Form (WV-103) -Used to retire equipment with an acquisition cost of $1,000 or less. If the equipment is on the WVFIMS Fixed Assets System, regardless of the acquisition cost, do not use this form. Use the WVFIMS retirement procedures instead.
