Report a Security Incident 

If you would like to report an Incident, simply click here or download this form and an Information Security Analyst will be in touch with you ASAP.

Be prepared to provide the following information:

  • Name
  • Phone Number
  • Agency
  • Cell/Pager
  • Physical Location of Incident (include building number, room number, etc)
  • Date and Time Incident Occurred
  • Date (mm/dd/yy)
  • Time (hh:mm:ss)
  • Is the Incident still ongoing? (yes/no)
  • Enter a brief summary of the reported incident.  If known, enter what you think has or is happening. (Examples: computer or network outage, compromised (hacked) system, stolen equipment or data, etc.)
  • Briefly describe the impact of the reported incident.  If known, include the number of affected critical systems, computers, networks, users and/or agencies.
