Submit one form per billing account.
Purpose: Use this form to request Voice over IP (VOIP) services.

VOIP services may involve coordination with agency IT staff.

The following guidelines outline the procedures for billing timelines and the process for contesting specific charges. Please ensure all payments and dispute notifications are submitted within the required windows to avoid penalties.
Agencies are responsible for adhering to the standard billing cycle to ensure continuous service.
Invoicing: Agencies receive a billing statement within 30 days of the receipt of detailed charges.
Payment Window: Each statement provides a 30-day window for payment.
Submission: Payments must be submitted to the Telecommunications Services Payment and Reserve Fund by the specified due date.
If an agency contests a portion of its statement, the following protocol must be followed. Note that full payment is still required by the deadline regardless of a pending dispute.
Deadline: A written notice of dispute must be submitted within 30 days of receiving the statement.
Submission Process:
Download and complete the Telecommunications Billing Inquiry Form.
Email the form and supporting documentation (including the full invoice) to: OTTelecomDisputes@wv.gov.
Note: Incomplete forms will be returned for correction.
Required Documentation: Include the service type (e.g., VOIP, SIP), the specific circuit number, the exact dollar amount, a clear explanation, and the full invoice as an attachment.
Failure to submit payment by the required due date will result in mandatory recovery actions.
Account Sweep: The Secretary of the Department of Administration may sweep the agency’s account to recover the full outstanding amount.
Late Penalty: An additional penalty of 3% of the statement amount will be assessed for all late payments.
